Important Booking Information:

When creating a patient profile for your child, use your child’s information when completing the form.

Please choose the appropriate service you wish to receive from the menu of services.

If you are booking appointments for additional family members, create a separate profile for each person using distinct usernames.

To join the waitlist, please also schedule an appointment. If an earlier time becomes available, you’ll be notified and may choose to keep your original appointment as a follow-up if needed.

If your needs are urgent, please call our office or include a note in the message area—we’ll do our best to accommodate you.
